La travagliata storia del Ticagrelor, ora diventa anche materia di indagine per il Dipartimento di Giustizia americano.
6 NovMolte riviste hanno pubblicato la notizia (diffusa anche da un portavoce ufficiale della casa farmaceutica produttrice, AstraZeneca) secondo la quale il dipartimento di giustizia Americano sta acquisendo notizie in merito allo studio Platelet Inhibition and Patient Outcomes (PLATO) presentato nel 2009. US Department of Justice Investigating PLATO Ticagrelor Trial
In questo studio si dimostravano i vantaggi del Ticagrelor sul Clopidogrel in termini di mortalità, tanto da suscitare l’entusiasmo del presidente dell’ American College of Cardiology, Doug Weaver (Henry Ford Health System, Detroit, MI), che dichiarava: “I think it’s the best result we’ve seen since aspirin, which was the last time we saw an antiplatelet drug with a mortality benefit. It could not have come out better. I really do think it’s going to surpass clopidogrel and pass prasugrel–it looks safer than prasugrel. In my own personal opinion, clopidogrel is too expensive for what you get. This is much, much better.”
Da allora la FDA si è comunque presa un lungo periodo di tempo prima di approvare l’uso del farmaco, tanto che nel 2010 non vi era ancora l’approvazione al suo utilizzo, rilevando le molte perplessità in merito ad alcuni aspetti dello studio PLATO riguardanti sia il disegno, che l’analisi dei risultati dello studio stesso (riassunti in questo editoriale: Paradoxical excess mortality in the PLATO trial should be independently verified).
Nel 2011, anno della definitiva approvazione del Ticagrelor da parte della FDA, Astra Zeneca si è vista “costretta” a condurre un nuovo trial in merito all’uso del Ticagrelor (PEGASUS), che al momento ha concluso la fase di reclutamento, ed i cui risultati sono attesi nel 2014, proprio volto a fugare i dubbi che PLATO aveva suscitato.
Astra Zeneca in effetti nega alcun nesso causale tra i rilievi emersi sullo studio PLATO e l’inizio del nuovo studio, negando che la FDA avesse ordinato il supplemento di informazioni.
Fatto sta che a due anni di distanza anche il Dipartimento di giustizia americano si occupa, aprendo un’indagine civile, dello studio PLATO, e la vicenda del Ticagrelor diventa sempre più complessa.
