Ketamine use in Traumatic Brain Injury: Pool result are here!

Some weeks ago we pubblished this post:

I’m deeply convinced that Ketamine use in TBI is safe, and that the evidences against his use in patients with TBI are based on old and small case studies affected by major limitations.
The recent evidences that shown as Ketamine doesn’t race ICP, despite increasing MAP (thank god!), are based on small but well done trials and are as good (or even more) than the previous ones.
I’m also convinced that the utlity of Ketamine in sedation of severe injuried patients so much outweight the so lightly evidenced controindication in TBI, that his use is still mandatory in trauma patients who need emergency sedation.

    Sottoscrivo a pieno. Direi che in Italia ove generalmente l’alternativa è il propofol o il midazolam la Ketamina è il farmaco che offre il miglior profilo per il trauma compreso quello cranico.
    Personalmente la utilizzo pressochè esclusivamente per l’induzione.
    Nella mia aneddotica esperienza credo che altre due cose che riguardano la Ketamina vadano un po’ limate.
    1)dosaggi: per la forma racemica il threshold dissociativo è 1mg/kg nella maggior parte dei pazienti ma non in tutti. Alcuni sono pienamente dissociati anche a dosaggi di gran lunga inferiori (0.4-0.5 mg/kg) e non parlo solo dei pazienti critici (shock, ipossia etc.)
    2) la preservazione del drive respiratorio: l’apnea è estremamente rara se la Ketamina viene somministrata lentamente (nell’arco di un minuto). Tuttavia raramente capita, anche senza considerare condizioni cliniche estreme, che pazienti, apparentemente sensibili, abbiano degli episodi di apnea anche protratti
